If you want to be part of the only safety net hospital in Nassau County, we have an amazing opportunity for a full-time Nurse Assessor/Case Manager. You will work at the Nassau County Department of Social Services, performing UAS assessments and case management for Medicaid recipients who live in Nassau County and are seeking/receiving home care services.
