Job Description
Position Overview:
The Clinical Manager is responsible for managing, supervising, coordinating, evaluating, and developing consumer care to ensure quality care delivery and appropriate case management within the Agency’s scope of services and policies, along with state, federal, and local laws/regulations.
Essential Job Functions:
- Supervises or manages, directly or through qualified subordinates, all personnel who provide direct consumer care
- Comply with all state regulations related to consumer visits, evaluations, and assessments and maintain and adhere to Agency procedure and consumer care policy manuals
- Coordinates consumer care service which includes making home visits
- Sets or adopts policies for, and keeps records of criteria for admission to service, case assignments, and case management
- Establishes and conducts an ongoing quality assurance program according to State Regulation and Agency policy
- Develops and manages consumers’ plans of care
- Develops working relationships with other health care professionals in the community and families to identify resources available and to ensure access of information to consumers
- Coordinates and ensures that the consumer is well serviced through the assignment of services and skilled and unskilled direct care staff whose skills and abilities meet their individual needs
- Provides training/competencies for direct care staff, when appropriate
- Ensures accuracy, completeness, and timeliness of documentation in accordance with Agency policies and procedures, regulatory requirements, and industry standards
- Reviews Agency policies and procedures and recommends changes or revisions as needed
- Provides educational programs and information regarding appropriate documentation practices
- Provides leadership to the team and supports staff in identifying Agency/consumer needs and opportunities for quality improvement
- Clinical on-call duties
- Follows Open Systems Healthcare’s Policies and Procedures
- Performs other duties as assigned by OSH
Required Knowledge, Skills, and Abilities:
- Able to work in a fast-paced work environment
- Ability to effectively communicate externally as well as internally with all levels of the organization
- Focus on Consumer Needs: Anticipate, understand, and respond appropriately to the needs ofinternal and external customers to meet or exceed their expectations within the organizationalparameters
- Ability to handle confidential information in compliance with HIPAA

- Public relations ability, interpersonal skills, and professional telephone manner

Required Qualifications, Education, and Experience:
- Graduate of an accredited school of professional nursing. Registered Nurse required; BSN preferred
- Minimum 1 year of case management experience as an RN; home care experience preferred
- Basic understanding of Microsoft Office
- Current and valid RN license
- Evidence of Tuberculosis screening that meets CDC guidelines
- Current and valid CPR certification
- Must have a reliable form of transportation
Working Conditions and Physical Demands:
- The employee is occasionally exposed to a variety of conditions at consumer sites
- 50% or more travel possible depending on branch and need
- Travel is primarily local during the business day, although some out-of-area and overnight travelmay be expected
- Ability to perform tasks involving physical activity, which may include light/medium moving andextensive self-positioning
